2. Waivers

All registrants sign The Forum’s waiver when creating their WellnessLiving Account. If you have a class specific waiver, that you would like added as a special note when participants register, please ensure to add it when submitting your new class request form.

3. Opening Procedures **Checklist is found on brown clipboard behind counter – please complete 

Opening Procedures

  1. Park and Enter at Rear Door. 
  2. Disable Alarm System (code provided)
  3. Turn on Main Studio Lights
  4. Turn on Bar, Lounge and Entrance Lights
  5. Turn on Fridge Lights. If applicable, turn on Ice Machine & Coffee Machine.
  6. Visual Facility Check Completed from Upstairs through to Main Studio. Ensure Tidiness and Cleanliness. 
  7. If applicable, turn on Black Ipad. 
    1. Code 123456
    2. Square App utilized for purchases
  8. If applicable, turn on Main Computer
    1. Password 2323
  9. Turn on White Ipad
    1. Open Mindbody Check in App and place on stand
  10. Use Sonos app to turn on music
  11. Unlock Front Door

Staff Only

  1. Check Phone Messages 
    1. Dial 403-848-1605
    2. Password 2323
    3. Record Messages and Distribute appropriately
  2. Complete Inventory Checklist.

Closing Procedures

  1. Lock Front Door
  2. Turn off Coffee Machine
    1. If you used;
    2. Empty Coffee Grounds
    3. Wipe Surfaces
  3. Wipe Down Surfaces
    1. Counter tops
    2. Kegerator
    3. Bar Top
    4. Benches, Tables
  4. Plug Black and White iPad into Chargers
  5. If Applicable, close any running computer programs and put Computer in Sleep Mode
  6. Receipts from day into ‘Current’ Month slot in Accordion Binder
  7. Turn off Fridge Lights, Counter Lighting
  8. Sweep Main Studio and Lounge Space
  9. Complete Video Check Of Facility
    1. Upstairs Bathroom 
      1. Tidy
      2. Empty Garbage if needed
      3. Turn off Lights
    2. Upstairs Studio 
      1. Tidy
      2. Turn off lights
    3. Stairs
      1. Turn off Lights
    4. Front Entrance
      1. Tidy
      2. Anything left behind? Place in Lost and Found
    5. Lounge Space
      1. Tidy
      2. Wipe Down Surfaces if needed
    6. Bar Space
      1. Wipe Down Surfaces
      2. Ensure no Beer/Kombucha left on counter
      3. Empty Garbage if needed
    7. Main Space
      1. Sweep Floor with Blue Broom
      2. Tidy
    8. Main Bathroom
      1. Tidy
      2. Empty Garbage if needed
  10. Turn off Sonos System
  11. Turn off Lounge, Bar Lights
  12. Turn off Main Space Lights
  13. Set Alarm. All doors must be closed to engage the alarm. Lock Door as you leave.

4. Safety

User Safety Checks & Policies

Prior to use of any Aerial Apparatus you must complete Safety Checklist. Please read below detailed explanation of how to complete Safety Check for each Apparatus. This is mandatory for usage. If an Apparatus fails Safety Check do not use it. 

Aerial Silks

Each Silk is denoted in Safety Binder by Colour. Find appropriate Silk in Safety Binder and Complete Checklist. 

  1. Ensure weekly checks are completed. 
  2. Check fabric for any flaws, rips, tears, runs. 
  3. Spin Apparatus multiple times in all directions to ensure the swivel moves smoothly. 
  4. From the ground look up to check the carabiner. Ensure it is closed – Flag should face down. 

*If there is any abnormality report to Forum Staff and mark as Out of Service. Do not use it.

Aerial Hoop

Each Hoop is denoted in Safety binder by a number tagged on Carabiner. Find appropriate Hoop in Safety Binder and Complete Checklist. 

  1. Ensure weekly checks are completed.
  2. Check hoop for any visible flaws.
  3. Spin apparatus multiple times in all directions to ensure the swivel moves smoothly. 
  4. From ground look up to check carabiner. Ensure it is closed – Flag should face down. 

*If there is any abnormality report to Forum Staff and mark as Out of Service. Do not use.

Trapeze

Find Trapeze in Safety Binder and Complete Checklist. 

  1. Ensure weekly checks are completed.
  2. Check trapeze for any visible flaws. Ensure dressing is not coming apart in any location. 
  3. From the ground look up to check the carabiner. Ensure it is closed – Flag should face down. 

*If there is any abnormality report to Forum Staff and mark as Out of Service. Do not use it.

Rope

Each Rope is denoted in Safety binder by a number tagged on Carabiner. Find appropriate Rope in Safety Binder and Complete Checklist. 

  1. Ensure weekly check was completed
  2. Check rope for any visible flaws.
  3. Pull and rotate rope multiple times in all directions to ensure the anchor moves smoothly. 
  4. From the ground look up to check carabiner. Ensure it is closed – Flag should face down. 

*If there is any abnormality report to Forum Staff and mark as Out of Service. Do not use it.

Rings

Find Rings in Safety Binder and Complete Checklist. 

  1. Ensure weekly check was completed
  2. Check wooden rings for any visible flaws, cracks and fractures.
  3. Check straps for any visible flaws, snags, rips and tears.
  4. Check ratchet to ensure it is closed. Pull hard once the visual check is completed. 

*If there is any abnormality report to Forum Staff and mark as Out of Service. Do not use it.

Pole

Each Pole is denoted by a number. Find the number at the top of the pole. Find appropriate Pole to Complete Checklist. 

  1. Ensure weekly check was completed
  2. Complete Visual Check of Pole for any damage, cracks and fractures.
  3. Grip Pole and pull sharply. Pole should not move. 
  4. If Pole does not move carefully, complete a spin with at least one foot on the ground. 

*If there is any abnormality or Pole moves report to Forum Staff or ask for assistance to tighten. If assistance is not available, mark as Out of Service and do not use it.
